Collaborative Therapy - Intensive EMDR
You might be wondering if Intensive Eye Movement Desensitisation and Reprocessing (EMDR) would be helpful for your clients. Maybe your client is making great progress in sessions, but there are some experiences that they are struggling to move pass and you want to try something different to unblock them?
What is intensive EMDR?
Intensive EMDR delivers treatment in short period of time, with multiple EMDR sessions per day. New research shows that intensive therapy for trauma and stress can be extremely effective in reducing symptoms.
You can read more about how sessions might look like and results from this article published in the European Journal of Psycho-traumatology by Bongaerts et al., 2021.
How does it work?
Typically, interventions take place online at agreed points during the day on up to three days during the course of a week in which particular goals are worked on based on specific unresolved memories.
It is recommended that a full week around the arranged dates is taken aside to focus on the programme. This means stopping regular activity that occupies one’s day/week (i.e., work and other related responsibilities) and instead adhering to an agreed plan of activity. It is like a week of holiday!
Please, bear in mind that this might vary slightly for each person. Intensive EMDR is bespoke and in-person and online services are available. Please, enquire below.
How does Intensive EMDR work alongside regular sessions?
Intensive EMDR usually works very well alongside regular sessions, where a trusted alliance is already in place and support is available before and after intensive work.
Coordination can easily take place via a brief conversation between the regular practitioner and Dr Lopez as well as email correspondence.
Regular sessions would usually be paused the week before and during the intensive treatment week. Please, be aware that there might be some variations from case to case.
Is EMDR intensive for everyone?
While many clients are good candidates for an intensive, some may do better with an ongoing therapeutic relationship.
- Clients with long-term childhood abuse or neglect, and/or have had years of traumatic events, are usually not advised to do an intensive treatment. In some cases, exceptions are made for those currently under the care of a trusted provider.
- Clients with this type of history can make progress on particular parts of events. In many cases, they need to continue with their regular therapist following the intensive treatment.
- Clients who are unlikely to be able to engage and make progress in this treatment context might not be referred.
Case Study
When I first met JJ, they were in their mid-40s and explained that their experiences of being bullied at school were still being triggered in their adult relationships both in personal and professional contexts. They described a degree of awareness about their habitual interpersonal patterns but said that did not know how to stop these patterns from impacting on their interactions with others in an undesirable way. As a result, they said that they inevitably found themselves feeling unwanted and flawed in relation to others and hopeless about their capacity to change these outcomes in the future. This was preventing them from maintaining healthy adult intimate relationships.
JJ’s narrative indicated that they were habitually guiding themselves by early life/old scripts leading them to think negatively about themselves and acting in a preset manner every time they noticed certain physical reactions in interactions with significant others. It became apparent that JJ’s earlier fears, a sense of unsafety, and lack of predictability, and those denied needs of the self that had been so very well learnt and protected kept playing an important role and preventing new and more functional strategies to become habitual. These automatic processes would get activated no matter their relevance whenever the right conditions met for them in personal and professional situations.
Following on from a resourcing phase, where JJ developed internal and external strategies that helped them regulate emotionally, they reprocessed the most salient experiences from their childhood that kept interfering in her adult interactions with others. These included specific memories of being bullied at school at the ages of 8, 10 and 13. JJ also reprocessed other associated memories that preceded these during therapy.
At follow up JJ advised that they felt as if “blinkers had been removed” from their eyes. They had observed how some negative thoughts persisted within some interactions with others, they did not feel compelled to act on them and instead felt freer to behave in ways that were consistent with their present needs. JJ reflected on their increased sense of trust on themselves and hope about their capacity to affect change in the future.
